Please follow this simple procedure. Once you have submitted a signed Resource Agreement Form, provided below, you will be able to access the Image Files for Before/After Photos, Testimonials, Company logos, etc. for use on your website.

1. Complete and sign the Vivier Image File Resources Agreement Form.

2. Fax the form to Vivier Pharma Inc. at 1-450-455-7491 or 1-866-455-7491 for approval.

3. Upon verification of eligibility, your request will be processed and you will receive an email Approval notification with a Username and Password giving you access to the Image files.
